Global surgery leap: a working prototype for the architecture we must build

Year: 2026
Type: editorial
Abstract: In her January 2026 Viewpoint in The Lancet, Dr Sania Nishtar, CEO of Gavi, advocated for a structural transformation of the current global health architecture, which she called the Global Health Leap.1 In it, she proposed four priorities (merger at the last mile, global public goods, large-scale country impact and fragile contexts) and four principles (country-centricity, national self-reliance, focused mandates and finite institutional lifespans).1 The Leap offers an opportunity to address existing high-priority challenges in surgical care delivery across low- and middle-income countries, including financing, indicator-tracking, noncommunicable disease (NCD) management, fragile-context care and constrained local ownership.
