A study of the cost and its contributing factors in Chinese patients with chronic thromboembolic pulmonary hypertension undergoing pulmonary endarterectomy

Abstract: Chronic thromboembolic pulmonary hypertension (CTEPH) is a progressive disease characterized by pulmonary artery lumen obstruction and vascular remodeling, leading to right heart failure with high morbidity and mortality if untreated. Pulmonary endarterectomy (PEA) is the first-line surgical treatment for CTEPH, but it is a high-risk, technically demanding procedure requiring multidisciplinary collaboration, resulting in substantial medical resource consumption. However, there is a lack of studies on the direct economic burden and its influencing factors of PEA in Chinese CTEPH patients, which hinders rational healthcare resource allocation and cost management. A total of 100 CTEPH patients who underwent pulmonary thromboendarterectomy at a Beijing hospital over the prior seven years were included. Descriptive analyses and a generalized linear regression model were employed to explore the patients’ medical costs and the contributing factors for their medical burden. Gender, age and duration of PEA operation had no effect on the medical burden in patients. Different types of medical insurance had no effect on the total inpatient expense but had an effect on the out-of-pocket expense in patients. The duration of postoperative intensive care was the main factor affecting the medical burden of patients. Each additional day of postoperative intensive care would increase the total hospitalization expense by ¥14,790.32($2,171.22)., the out-of-pocket expense by ¥11,990.85($1,760.26), and prolong the actual length of hospitalization by 0.88 days. The postoperative recovery of patients has a significant impact on cost and length of hospital stay, and the structure of hospitalization costs needs further optimization. Patients have accumulated too many numbers of times of hospitalizations and have too high proportion of self-paid expense. It is necessary to promote centralized purchasing of surgical consumables and enhance perioperative care to reduce therapeutic consumable use, as well as improve the coverage of medical insurance.
