Abstract: Background: Diabetic kidney disease (DKD) is a major microvascular complication of type 2 diabetes, and dysregulated lipid metabolism contributes to its progression. PLA2G12B is involved in lipid metabolism, yet its subcellular localization and detectability in human serum remain controversial. Clinical evidence regarding the association between circulating PLA2G12B and DKD is still limited. Aim: To explore the association between circulating PLA2G12B level and the probability of DKD and to perform exploratory analyses of lipid profiles across different disease statuses. Methods: A total of 230 participants were included in this cross-sectional study. Baseline clinical information, lipid parameters and serum PLA2G12B concentrations were measured. Multivariate logistic regression was applied to explore the association between serum PLA2G12B and DKD. Loess smoothing curves were used to visualize the changing trends, and stratified analyses were performed to evaluate potential interactions. Results: Elevated circulating PLA2G12B was independently associated with increased possibilities of DKD with a distinct threshold effect. Stratified subgroup analyses confirmed a consistent positive correlation across most subgroups and a significant interaction effect was detected for age tertiles. The trends between PLA2G12B and lipid/insulin resistance/atherogenic parameters across distinct disease stages highlight its stage-dependent role in lipid metabolism of diabetes. Conclusion: Our study identifies an association between circulating PLA2G12B and DKD among patients with type 2 diabetes. These observational findings provide clues for future research exploring biological pathways relevant to diabetic kidney injury, and further studies are required to characterize the clinical relevance of circulating PLA2G12B.
