An Unusual Vascular Proliferation Presenting a Clinicopathological Challenge

Abstract: Uncommon cutaneous vascular lesions can present with overlapping clinical features, creating a diagnostic challenge. Histopathological examination is often essential to establish the diagnosis and exclude malignant mimics. We describe a patient with multiple erythematous papules and nodules affecting the head and neck. The clinical differential diagnoses included several benign and malignant vascular lesions. Histopathological examination demonstrated a proliferation of vascular channels lined by epithelioid endothelial cells with characteristic intraluminal protrusion, accompanied by a mixed inflammatory infiltrate rich in eosinophils. This case highlights the importance of collaboration between the dermatologist and dermatopathologist in the assessment of unusual vascular lesions for accurate diagnosis. The patient subsequently underwent surgical treatment with punch excision for smaller lesions and shave excision with cautery for larger lesions, with satisfactory clinical improvement and no reported recurrence to date. However, this condition can be challenging to treat and further research is needed to establish more effective and standardized treatment strategies.
