Mechanical Thrombectomy of a Combined Persistent Primitive Trigeminal Artery and Basilar Artery Occlusion: A Case Report and Literature Review

Abstract: Objective: Persistent primitive trigeminal artery (PPTA) is a rare anastomosis between the internal carotid artery and the posterior circulation, most commonly with the basilar artery (BA). It has been associated with neurovascular diseases such as cerebral hemorrhage and ischemic stroke. Its occlusion in the case of ischemic stroke presents multiple challenges, both diagnostic and therapeutic, as the modified vascular anatomy may complicate the initial imaging diagnosis and endovascular treatment with mechanical thrombectomy (MT). Case Presentation: We report here the case of a 64-year-old woman presenting with an altered conscious status and bilateral mydriasis due to an occlusion of a PPTA and the distal part of the BA, which was treated with MT. Complete recanalization was obtained using combined contact aspiration and stent retriever thrombectomy through the PPTA. Conclusion: PPTA is a rare vascular anatomical variant whose knowledge is important in the setting of stroke. In this setting, MT appears to be a feasible, safe, and effective technique compared to other proximal sites of occlusion.
