Advancements in the Measurement of Orthorexia Nervosa After the 2022 International Consensus: A Systematic Review of Psychometric Evidence
Year: 2026
Type: review
Study design: Systematic Review
Abstract: Orthorexia Nervosa (ON) involves excessive preoccupation with healthy eating, rigid dietary control, emotional distress, and functional impairment. Research progress has been limited by inconsistent diagnostic criteria and heterogeneous ON assessment instruments. The 2022 international consensus report defined core diagnostic domains for ON, providing a framework for contemporary scale development. Novel ON assessment instruments have since emerged, yet their psychometric quality and conceptual alignment with consensus-based criteria have not yet been synthesized. A PRISMA 2020-guided systematic review was conducted. PubMed, Scopus, and Web of Science Core Collection were searched for studies published between January 2023 and January 2026 reporting the development of ON assessment instruments. Five eligible studies were included. Sample sizes ranged from 418 to 978 participants. Two instruments assessed temporal stability, and two proposed preliminary cut-off scores. All instruments reported factor-analytic validation and acceptable-to-excellent internal consistency (α/ω ranging from .77 to .94). Domain mapping demonstrated uniform assessment of preoccupation with healthy eating. Post-consensus ON assessment instruments demonstrate advancing psychometric sophistication but remain conceptually diverse.
