Abstract: Acute respiratory tract infections (ARI) are still one of the main causes of pain in toddlers, and preventive behaviors at the family level play an important role in suppressing its occurrence. This study aimed to analyze the factors that affect the prevention behavior of ISPA in toddlers in Galang District, Deli Serdang Regency. This study used a quantitative approach with a cross-sectional design involving 100 respondents. Data were analyzed univariately, bivariately with chi-square test, and multivariate with binary logistic regression. A total of 67.0% of respondents had poor ISPA prevention behavior. Multivariate analysis showed that education (OR = 2.94; 95% CI: 1.35–6.39; p = 0.007) and attitude (OR = 9.82; 95% CI: 2.44–39.63; p = 0.001) had a significant effect, while knowledge was insignificant (OR = 1.97; 95% CI: 0.50–7.74; p = 0.334). The model had a Nagelkerke R² value of 0.538 and a classification accuracy of 83.0%. Attitude was the most dominant factor influencing the prevention behavior of children under five. Health promotion interventions need to be directed at the formation of positive maternal attitudes and strengthening family health education.
