Telitacicept treatment in IgA nephropathy: A retrospective study

Year: 2026

Study design: Observational Study
Abstract: Objectives Immunoglobulin A nephropathy (IgAN) is the most common primary glomerular disease and is the leading causes of chronic kidney disease (CKD). New therapies are urging in this area. The effectiveness and safety of telitacicept, a new potential drug targeting B cell-activating factor (BAFF) and a proliferation-inducing ligand (APRIL), in real-world management of IgA nephropathy requires further investigation. This study was designed to evaluate the efficacy and safety of telitacicept in patients with IgA nephropathy from our center. Methods This retrospective analysis included 21 primary IgAN patients receiving telitacicept. Baseline characteristics and follow-up data were collected. We assessed changes in proteinuria, complete remission rates, change of eGFR. Adverse events occurred during the treatment were collected. Results Telitacicept treatment resulted in substantial proteinuria reduction (76% decrease from 1.67 to 0.4 g/24h, p<0.001), with 29% achieving complete remission. Renal function remained stable throughout follow-up. Notably, some patients with advanced kidney disease (eGFR <30 mL/min/1.73m 2 ) showed preserved renal function. Safety monitoring revealed minor adverse events without treatment discontinuations. Combined therapy also revealed a satisfied efficacy and safety. Conclusion Telitacicept demonstrates robust proteinuria reduction and renal function preservation in real-world IgAN treatment, while maintaining a favorable safety profile even in advanced disease stages.
