A Review of Surgically Treated Cases With Intermittent Claudication

Abstract: Between January 1976 and December 1985, 115 patients with arteriosclerosis obliterans (ASO) and 12 with Buerger's disease (TAO) underwent reconstructive surgery for intermittent claudication, an indication for operation. These cases were reviewed. The mean age was 61.8 years in ASO and 41.7 years in TAO. Although there were no preoperative complications in TAO, there were 43 cases (37.4%) with heart disease, 15 (13.0%) with diabetes mellitus, and 13 (11.3%) with cerebrovascular insufficiency in ASO.Eighteen patients with ASO died postoperatively, including two operative deaths. Four patients died of myocardial infarction, and five died of malignant tumors. Occlusion of the implanted graft occurred in 22 limbs (11.3%) in ASO, three cases of which received major amputations. Graft occlusion occurred in three limbs in TAO.As heart disease which results from arteriosclerosis is the main cause of postoperative and late deaths in ASO, the operative indication should be reached after careful preoperative assessment in each case, and intensive care should be provided during and after surgery. Therefore it is considered that the operative indication of intermittent claudication should be limited to the severe cases, because the long-term patency rate of bypass grafts to the crural arteries has not been good.
