Abstract: Introduction: The adequate selection of patients to use non-invasive mechanical ventilation is important to its success. In pursuit of the best practice, assistance protocols have been implemented to optimize patient care. Objectives: To assess whether the use of a non-invasive mechanical ventilation assistance protocol, compared to the use without a protocol on patients in an intensive care unit, increases the success of the technique. Patients and Methods: Cohort study with background controls that followed up on patients that required non-invasive mechanical ventilation during their hospitalization. Patients who presented contraindications were excluded. The ventilation characteristics, medical chart and cardiorespiratory variables of both groups were collected. Results: In the study 93 patients were included in the protocol group and 48 in the group without protocol. Chronic obstructive pulmonary disease, cardiogenic acute lung edema and difficult weaning were the most frequent causes for use in the protocol group; whereas in the group without protocol, they were chronic obstructive pulmonary disease, pneumonia and acute pulmonary lesion/adult respiratory distress syndrome. The mean hospitalization time was 9 versus 13.5, the mean ventilation time was 2 versus 3.5, and the mortality rate of 17.2% versus 56.3%, was lower in the group with protocol.
