Multimodality imaging with US, CT, and MRCP of biliary ascariasis: A case report

Study design: Case Report
Abstract: The imaging diagnosis of biliary ascariasis has evolved in parallel with the development of diagnostic radiology. This case report describes multimodality imaging findings with ultrasound (US), simple computed tomography (CT), and magnetic resonance cholangiopancreatography (MRCP) for the diagnosis of biliary ascariasis in a 7-year-old girl with abdominal pain, jaundice, fever, vomiting, and bowel movements with Ascaris lumbricoides.Abdominal US showed a trilaminar, tubular, hyperechoic image with bile duct dilatation and parasite movement inside the gallbladder.A simple abdominal CT showed nonspecific findings with multiple round hyperdense images in axial sections, and coronal reconstruction showed tubular and semilunar images inside the gallbladder. On MRCP, the parasite was seen as an endoluminal linear hypointensity in the common bile duct and common hepatic duct with obstruction and dilatation of the intrahepatic and extrahepatic bile ducts.A tubular, trilaminar ("spaghetti") image consistent with intestinal ascariasis was observed in the redundant sigmoid. This is the first case report from Mexico demonstrating that the accuracy of US and MRCP in diagnosing biliary ascariasis is comparable. We do not recommend multimodality imaging in most cases, so US examination has a high accuracy in the diagnosis of biliary ascariasis.
