Abstract: Background and Objectives: The data on prevalence of heart failure, its determinants, and treatment practices in patients with rheumatic fever (RF) rheumatic heart disease (RHD) are limited. We report the prevalence of heart failure, its risk determinants in patients with RF/RHD using prospective tertiary care hospital-based registry data from northern state of India. Materials and Methods: The data of sociodemographic, clinical characteristics, heart failure, and echocardiography-based assessment of severity of valvular dysfunction and left ventricular (LV) systolic function were systematically recorded prospectively in 2840 consecutive patients of RF/RHD to estimate. The patients with New York Heart Association (NYHA) functional class 11 to 1V were labeled as heart failure. Results: The mean age of the study population was 41.1 years, affecting predominantly females, from rural area with lower levels of education. Heart failure was prevalent in 77.1%, 95% confidence interval (75.9%, 78.6%). The moderate to severe mitral stenosis odds ratio with 95% confidence interval 2.04 (1.67, 3.44), mixed mitral valve disease 1.63 (1.19, 2.25), left ventricular ejection fraction 0.98 (0.97, 1.00), atrial fibrillation (AF) 1.95 (1.3, 2.9), pulmonary artery hypertension 1.88 (1.38, 2.55), history of valve replacement 0.56 (0.38, 0.83), and level of education 0.86 (0.75, 0.97) had significant association with heart failure. Conclusion: Heart failure is highly prevalent in patients with RF/RHD. Severity of valvular stenosis and LV dysfunction were the important determinants. Surgical replacement of valves and higher education status were associated with lower prevalence of heart failure.
