Abstract: This case report aims to describe endodontic treatment of a maxillary second molar fused with a supernumerary tooth which is a very rare case in the literature. 27-year-old patient was referred to the clinic with severe tooth pain. After clinical and radiographic examination pain was defined in fused left maxillary second molar (Tooth 2.7) with a supernumerary tooth. CBCT examination showed that fusion occurred mostly at the cementum and a little at the dentin level of the distal root and supernumerary tooth, 4 root canals at tooth 2.7, and 2 root canals at supernumerary tooth (Vertucci class 5). Furthermore, caries occurred at the fusion site of the coronal region and periapical radiolucency at the distal root. Root canal opened under water coolant with standard access cavity at tooth 2.7 with a hole at supernumerary teeth. A path file (T-endo must tg, Dentac, Oncu Dental, Turkey) was used and the tooth was shaped to 25.04 taper rotary files (EndoART, Inci Dental, Turkey), filled with bioceramic root canal sealer (Bioserra, Dentac, Oncu Dental, Turkey) and gutta-percha (DiaDent Group International, Korea). In this case, tooth decay occurred because of insufficient cleaning of the fusion area, and decay caused complaints of the patient. Despite the complexity of the canal shapes, root canals were identified with the help of CBCT and completed in a single visit. Fusion of supernumerary tooth and maxillary molar is a rare case and CBCT is a helpful device for better understanding the complicated root canal morphology of the fused teeth.
