Chronic epididymitis as a sign of Zinner syndrome: Case report

Year: 2026
Type: article
Study design: Case Report
Abstract: Introduction: Zinner syndrome is a rare congenital anomaly of the urogenital tract, characterized by a triad of seminal vesicle cyst, unilateral renal agenesis, and ejaculatory duct obstruction. It can manifest as chronic epididymitis, although it is often diagnosed incidentally. Clinical Observation: We report two clinical cases: the first involves a patient with recurrent epididymitis, leading to further investigations that confirmed Zinner syndrome. The second case was an incidental finding during an abdominal ultrasound. Discussion: The diagnosis of Zinner syndrome relies primarily on imaging techniques such as ultrasound and MRI, which help identify the associated urogenital anomalies. Management depends on symptom severity: asymptomatic patients require regular follow-up, while symptomatic cases may need medical or surgical treatment. Conclusion: Although rare, Zinner syndrome should be considered in patients with unexplained chronic epididymitis or unilateral renal agenesis. Early recognition allows for appropriate management and improved patient outcomes.
