Abstract: Music box spiny keratoderma is a rare hyperkeratotic disorder affecting the palms and soles, which may present in familial or sporadic forms. Familial cases are typically observed at younger ages, whereas sporadic cases are more frequently seen in older patients. Sporadic cases of spiny keratoderma may be associated with underlying malignancies and systemic diseases. Therefore, a comprehensive systemic evaluation should be performed in all patients with spiny keratoderma. In this report, we present a newly diagnosed case of acquired spiny keratoderma. Although acquired spiny keratoderma has been associated with idiopathic pulmonary fibrosis, the patient's cutaneous lesions developed 2 years before the diagnosis of idiopathic pulmonary fibrosis. With this case, we aim to highlight the potential coexistence or sequential development of idiopathic pulmonary fibrosis in patients with spiny keratoderma.
