Abstract: TEVAR infection complicated by aortoesophageal fistula is rare and carries high morbidity. We report the case of a 71-year-old man with TEVAR infection caused by Mycobacterium haemophilum who underwent initial bridging TEVAR. Ten months later, he developed an aortoesophageal fistula and a suspected left aortobronchial fistula. Multidisciplinary surgical management included explantation of the endoprosthesis, rifampin-soaked Dacron reconstruction, resection of infected tissue including esophagectomy and left pulmonary wedge resection, and omental wrapping with an intercostal muscle flap. Postoperatively, recovery was favorable, and the patient’s blood cultures remained negative, supporting a curative approach in selected patients.
