Abstract: Introduction: Urdhwaga-Raktapitta is a disease with bleeding from the orifice above the neck. Ocular bleeding is considered as the Urdhwaga Raktapitta (~ an ayurvedic eye disease) as eye is a Navadwara (~nine orifices). In contemporary medical science, Retinal vein occlusion causes Vitreous hemorrhage. The treatment modality includes the anticoagulants, thrombolytics and hemodilution, intra-vitreal anti-Vascular Endothelial Growth Factor (anti-VEGF) injections, pan-retinal LASER photocoagulation, and pars plana vitrectomy. The effects of these drugs are uncertain and none of them is proven hundred percent effective. Patient information: Here we have presented a female patient having sudden painless vision defect with distant visual aquity1/60, Cystoid Macular edema, multiple bleeding points and multiple cotton wool spots. Materials and Methods: Considering the case as Rakta and Pitta vikara janya vyadhi; we managed the case with Shamana Aushadhi (~Conservative treatment), Virechana (~ Therapeutic Purgation), Netra Kriyakalpa (~Topical Ayurvedic ocular therapy) and various Murdhni Taila (Topical Ayurvedic therapy for head) procedures. Results: After twenty-third day of treatment, the Cystoid Macular edema was completely resolved, the number and size of the Hemorrhagic points and cotton-wool spots were reduced according to the Optical Coherence Tomography (OCT). The vision was also improved from 1/60- to 6/9. In addition, after the third month of treatment, the OCT suggests completely normalcy of Vision as distant visual acuity (DVA) 6/6. Along with the treatment, the patient was advised to maintain a stressless life and with simple diet. Conclusion: This case demonstrates that a structured, multi-modal Ayurvedic protocol, consisting systemic bio-purification (Sadyo Virechana), ocular therapies (Netra Kriyakalpa), and internal medications (Rakta-Pitta Shamana), can reverse profound vision loss from acute CRVO, achieving full visual acuity restoration (1/60 to 6/6) and resolution of edema within 90 days. The findings suggest this approach effectively manages retinal ischemia and vascular permeability by addressing Rakta and Pitta dosha vitiation, presenting a potential non-invasive alternative to anti-VEGF injections that requires further validation through clinical trials.
